The skylight was clearly the main source of illumination which is another difference to modern facilities which are usually lit by football stadium lights. Perhaps England used to be brighter back then as I'm fairly sure those four bulbs wouldn't quite do the job in the middle of November but then again its not as if the patients were hoping for life saving keyhole treatment.
